Checkout Intake Fields
Attach a CRM intake form to direct service checkout.
For direct service checkout, use a CRM intake form.
Attach an Intake Form to a Service
- Create the form in CRM > Forms.
- Open the service from Services > All Services.
- Select Settings.
- Choose Checkout intake form.
- Save and open the service's checkout link to review the fields.
The selector appears when forms are available or the service already has a saved form. Submissions are linked to the checkout project when one is created, or to the contact otherwise. Review responses in the form's Submissions.
Standalone Link Fields
The separate Services > Checkout Links form has Custom Intake Fields with text, textarea, select, and checkbox inputs. Those fields belong to the standalone link record and do not configure the service's direct checkout.
Current Availability
These settings can be saved, but the public purchase flow for standalone catalog and checkout-link records is not currently available. Saving an active record does not make its generated URL a working checkout.
For a single-service purchase, open Services > All Services, select the service, and use Copy checkout link in its header. Review the service checkout flow before sharing.
